Sheriff's TAKE ME HOME

The San Diego County Sheriff's Take Me Home Program allows you to register a loved one in a data-base for easy reference in case of wandering or other emergency. Law Enforcement has access to the information while they respond.

Health Care Directives

An Advance Health Care Directive enables individuals to make sure that their health care wishes are known in advance and considered if for any reason they are unable to speak for themselves. An advance directive also allows a patient to appoint a health care "agent" who will have legal authority to make health care decisions in the event that patient is incapacitated, or immediately upon appointment if the patient expressly grants such authority, known as "Power of Attorney" or POA.

Resource Guide

Caregiver Resource Centers provide support for family caregivers through asssessment, consultation, case planning, and support groups. Funded by grants and donations.

Networkofcare is an online library, service directory and more. Provided by Aging & Independence Services of San Diego County's Health & Human Service Agency, the site is a one-stop hub for information.

The coalition is a county-wide network of free and low-cost transportation services available to seniors. Our programs offer safe, reliable transportation provided by trained volunteers who care about seniors.

AARP has a number of online resources for caregivers including tipes, webinars, and guides.
